Maddie Holt

Service Lead / Support Coordinator

Maddie brings a positive and collaborative approach to her role, supporting participants, staff, and families to achieve meaningful outcomes. Through her leadership within accommodation and tenancy services, she is passionate about promoting independence, creating supportive environments, and ensuring participants feel safe, respected, and empowered in their homes.

Maddie is a kind, friendly, and enthusiastic team member at ACHIEVE Disability Services Pty Ltd, working in a dual role as a Support Coordinator and Service Lead. She is passionate about supporting people to make the most of each day and helping participants achieve meaningful outcomes. With a positive outlook and approachable nature, Maddie builds genuine, supportive connections with participants, families, and staff, encouraging individuals to explore their interests, build confidence, and work towards their personal goals.

As a Support Coordinator, Maddie works closely with participants and their families to help them understand and implement their NDIS plans. She supports participants to connect with the right services, coordinate supports, and build the skills and confidence needed to manage their supports more independently. Maddie takes a person-centred approach, ensuring each participant’s goals, preferences, and strengths remain at the centre of the planning process while helping them navigate the NDIS system and maximise the supports available to them.

In her role as Service Lead, Maddie oversees the operations and day-to-day coordination of ACHIEVE’s Supported Independent Living (SIL), tenancy, and accommodation services. This includes supporting participants living within shared and supported accommodation settings, ensuring supports are delivered safely, effectively, and in line with participant goals and individual needs. She works closely with support teams, participants, families, and stakeholders to promote positive living environments, encourage independence, and maintain high standards of care and service delivery.

Maddie’s Service Lead responsibilities also include overseeing staff support and guidance, assisting with service planning, monitoring participant outcomes, supporting tenancy stability, and helping ensure accommodation environments remain participant-focused, welcoming, and responsive. She plays an important role in maintaining quality supports while encouraging participants to develop life skills, increase independence, and actively engage within their homes and communities.

Maddie has experience supporting individuals with Autism Spectrum Disorder (ASD), Intellectual Disability (ID), Down syndrome, non-verbal communication, and mental health challenges. Her strong understanding of participant needs, combined with her warm and person-centred approach, allows her to build positive relationships and provide meaningful support across both of her roles.

To ensure ethical practice and compliance with NDIS guidelines, Maddie supports different participants in each role, and her dual responsibilities do not create a conflict of interest. She does not advocate for one service over another and ensures participants maintain full choice and control over the providers and supports they engage.

Maddie has a strong interest in music—particularly live music—along with reading, socialising, and sports, and is always open to exploring new activities and experiences with participants in a fun, engaging, and motivating way.

Maddie is inclusive, welcoming, and respectful of the LGBTQIA+ community, ensuring everyone she supports feels safe, valued, and respected. Having recently moved to Australia from the UK, she brings a fresh perspective, genuine enthusiasm for connecting with others, and has become a valued member of the ACHIEVE team through her dedication to participants, staff, and service quality.
