Achieve Disability Services offers Support Coordination to help you make the best use of your supports and get the most out of your NDIS plan. Support Coordination is a capacity-building service designed to help you understand your plan, connect with the right providers, and build the skills you need to coordinate your supports with confidence.
Our Support Coordinators take the time to understand your needs, preferences and goals. They’ll help you interpret your plan, explain funding categories and terminology, and guide you through the process of accessing community, mainstream and government services. You’ll also receive support in choosing and managing service providers, building your independence, and ensuring your plan continues to meet your needs as they evolve.
There are three levels of support coordination available under the NDIS:
Your level of funding for Support Coordination will depend on what is considered ‘reasonable and necessary’ to help you achieve your goals. It may be included in the Capacity Building budget of your plan. Where stated, you’ll receive the specified level of support coordination; otherwise, you can choose the level that best suits your needs.
Receive personalised help to understand and use your NDIS plan effectively to pursue your goals.
Build connections with NDIS providers, community organisations and government services to ensure your supports work together.
Explore all available options, stay in control of your supports, and receive help resolving issues with service providers if they arise.
Reflect on your experiences, refine your plan as needed, and access specialist support coordination if your situation is more complex.
