Maddie Holt

Service Lead / Mentor / Support Worker

Maddie brings a positive and collaborative approach to her role as Service Lead, providing leadership and guidance across ACHIEVE’s accommodation and tenancy services. She works closely with participants, staff, families, and stakeholders to promote independence, maintain high-quality supports, and ensure participants feel safe, respected, and empowered within their homes.

Maddie is a kind, friendly, and enthusiastic Service Lead at ACHIEVE Disability Services Pty Ltd. She is passionate about supporting people to make the most of each day and helping participants achieve meaningful outcomes. With a positive outlook and approachable nature, Maddie builds genuine and supportive relationships with participants, families, staff, and stakeholders, encouraging individuals to build confidence, increase independence, and work towards their personal goals.

As Service Lead, Maddie oversees the operations and day-to-day coordination of ACHIEVE’s Supported Independent Living (SIL), tenancy, and accommodation services. She plays an important role in ensuring participants receive safe, consistent, high-quality, and person-centred supports that reflect their individual needs, preferences, goals, and choices.

Maddie works closely with participants, families, support teams, and external stakeholders to promote positive and stable living environments. She supports the coordination of services within ACHIEVE’s accommodation settings, monitors participant needs and outcomes, and assists in ensuring supports continue to evolve alongside each individual.

A key part of Maddie’s role involves providing leadership, guidance, and support to ACHIEVE’s frontline teams. She assists staff with day-to-day service delivery, promotes clear communication and consistency across support teams, and helps ensure staff understand participant-specific requirements, routines, goals, and expectations. Her approachable leadership style allows staff to feel supported while maintaining a strong focus on accountability and quality service delivery.

Maddie also assists with service planning, participant transitions, tenancy stability, stakeholder communication, documentation, and continuous improvement across ACHIEVE’s accommodation services. She works to identify and address concerns as they arise and collaborates with the broader ACHIEVE management team to ensure services remain responsive, well-coordinated, and aligned with organisational and NDIS requirements.

Maddie has experience supporting individuals with Autism Spectrum Disorder (ASD), Intellectual Disability (ID), Down syndrome, non-verbal communication, and mental health challenges. Her understanding of diverse and complex support needs, combined with her warm and person-centred approach, enables her to build strong relationships and provide effective leadership across the services she oversees.

Central to Maddie’s approach is ensuring participants remain at the heart of their supports. She encourages choice and control, independence, community participation, skill development, and meaningful involvement within the home, while working with support teams to maintain environments that are welcoming, respectful, safe, and responsive to each participant.

Outside of work, Maddie has a strong interest in music—particularly live music—reading, socialising, and sports, and enjoys exploring new activities and experiences.

Maddie is inclusive, welcoming, and respectful of the LGBTQIA+ community, and is committed to ensuring the people she works with feel safe, valued, heard, and respected. Having moved to Australia from the UK, she brings a fresh perspective and genuine enthusiasm for connecting with others and continues to make an important contribution to ACHIEVE through her dedication to participants, staff, and the ongoing quality of our services.
